| [Dear/Hello/Hi/Good morning/Good afternoon/Good evening/Greetings] [Dr/Doctor/Miss/Mr/Mrs/Ms] [First Name] [Last Name],
| | Thank you for your partnership and your patients’ commitment to the apitegromab clinical program.
| | ONYX Clinical Trial Information
| | As a follow-up to our recent conversation, as of [date], ISEMBYLD™ (apitegromab-mstn) has been approved by the US Food and Drug Administration (FDA). Per the ONYX clinical trial protocol, patients who wish to continue with ISEMBYLD treatment will need to transition to commercially
available product once a path to access is determined. Patients may also need to complete an end-of-treatment visit, which will serve as their end-of-study visit, before receiving their first commercial infusion. Your Clinical Research Associate will be in touch about end-of-study/end-of-treatment protocol once insurance approval is in place.
| | Our priority is to ensure uninterrupted care for patients who choose to continue treatment with ISEMBYLD. We understand that insurance approval is needed before transitioning to commercial therapy, and Scholar Rock is committed to supporting you and your team throughout this process.
| | Next Steps: Scholar Rock Supports™ Enrollment
|
Enrollment in Scholar Rock Supports will enable us to help coordinate and support appropriate patients through the transition from the ONYX clinical trial. Our program can help patients with:
| •
| Insurance access and benefits support

| Financial support and copay assistance (for eligible patients)
| | •
| Infusion day coordination (at home, in hospital, or infusion center)
| | •
| Caregiver support and resources
