[Dear/Hello/Hi/Good morning/Good afternoon/Good evening/Greetings] [Dr/Doctor/Miss/Mr/Mrs/Ms] [First Name] [Last Name],
Thank you for your partnership and your patients’ commitment to the apitegromab Expanded Access Program (EAP).
As a follow-up to our recent conversation, ISEMBYLD™ (apitegromab-mstn) has been approved by the US Food and Drug Administration (FDA). Scholar Rock’s patient support team is ready to help you and your staff transition your patients who will continue on treatment from the EAP to the commercially available product. We encourage you to leverage our Scholar Rock Supports™ program to ensure your patients are supported throughout this transition.
The Transition Process
Upon approval, the FDA requires an expedient closure of our apitegromab EAP. Our priority is avoiding interruptions in care for children and adults in the EAP who wish to continue treatment with ISEMBYLD. We understand that patients will require insurance approval before transitioning to commercial product.
Next Steps: Scholar Rock Supports Enrollment
Enrollment in Scholar Rock Supports will enable us to help coordinate and support appropriate patients through the transition off the EAP. Our program can help patients with:
• Insurance access and benefits support
• Financial support and copay assistance (for eligible patients)
• Infusion day coordination (at home, in hospital, or infusion center)
• Caregiver support and resources
• Guidance on what patients can expect with treatment
The Scholar Rock Supports enrollment form must be signed by the prescriber and patient, caregiver, or legal representative. Patients can review, complete, and electronically sign the patient sections of the form via the Scholar Rock eConsent portal at srrksupportsconsent.com. Once completed, it will activate support by an experienced and dedicated team including an Rx Onboarding and Care Coordinator (ROCC).

We want to ensure patients feel supported throughout this transition. To help navigate coverage, affordability, and the transition to the first commercial dose, it's important to enroll patients in Scholar Rock Supports. Although not required for access, enrollment is necessary for the patient and/or caregiver to receive support from Scholar Rock.
I’ve attached a one-page, step-by-step patient transition guide for your reference. If you have transition questions, please contact [me/your ROCC]. To get started, please click the link below to download and print the Scholar Rock Supports enrollment form. Please read the instructions on the form for how to complete and return it via fax to 877-219-0016.
